Adult Flu Consent

We are now able to give parents a flu vaccine at their child's scheduled drive thru clinic appointment! (parent flu vaccine available ONLY at drive thru clinics for registered Annapolis Pediatrics' patients)

Parent flu vaccines are $30, to be paid at the time of service. If you elect to receive your flu shot through us, you understand that no insurance will be billed by Annapolis Family, LLC nor can you submit to your insurance for reimbursement as we are not credentialed with any insurance. Cash or Credit Card only

Adult Flu Consents will be provided at the flu clinic.
